### Proxy concurrency hotfix
- Treat the configured proxy connection count as one process-wide segment connection budget shared by all simultaneous downloads instead of multiplying it per file.
- Coordinate proxy leases across downloads, prevent concurrent reuse of the same list entry, and reserve the fixed API proxy from segment traffic.
- Preserve HTTP status codes returned by the download origin so reachable proxies receiving responses such as HTTP 503 are no longer reported as unavailable proxies.
